How to Apply

How to Apply - Medicaid Disability

Traditional Medicaid is a healthcare program for low-income individuals that offers medical care such as doctor visits, prescription drugs, dental and vision care, family planning, mental health care, surgeries and hospitalizations.

When you apply for Medicaid Disability, you  apply at your local DFR office but you will also need to apply at the Social Security Administration office to determine disability eligibility.  Once that has occurred, then Medicaid will be notified and you will be directed about insurance options.

You can apply on the internet, phone, or you can go to your local DFR office.

Click on the county map below to find your local DFR office or call 1-800-403-0864.
