How do I Apply

Report Changes

Call 1-800-403-0864 or visit the place where you applied to report changes.
Report all changes, such as:
  • new job
  • new address
  • new phone number
  • change in income
  • change in the number of people in your household
Recertify Each Year
Know the month and day that you need to renew your benefits also called redetermination date or recertification date. To recertify, complete the necessary paperwork and update income for the last 30 days. If you don’t know your recertification date, you can call 1-800-403-0864 to find out.
Know Your Health Plan
Know who to call for transportation services, questions about your benefits and choosing a doctor. Call 1-800-889-9949 to find out your plan name and have your questions answered.
Pay Your Part
If you have a monthly premium for Hoosier Healthwise or a monthly contribution for HIP, pay your premiums and contributions to avoid losing your health coverage.
Open and Read Your Mail
Open and read your mail from your health plan and from the State of Indiana.  This is the only way to get information to you.  Read it and take action.
